HOW DO I GET STARTED?

We prepared a video that explains how you can determine if the program is right for you. The 5 Steps outlined in the video will prepare you for enrollment into the program. Our care coordinators are here to assist you every step along the way. The care team will work with your physician to identify patients that would benefit from this service. If you are selected you may receive a phone call from a care coordinator to explain the program, assist you with enrollment, and provide you a roadmap to success.

CARE COORDINATORS

Your Care. Anywhere. (YCA) offers connected care services to help you navigate your healthcare journey. Our YCA care coordinators are there to assist you every step of the way. The care coordination team works with your physician to develop a personalized care plan to ensure you receive maximum benefits from the programs.

You will receive monthly telephone assistance in between your regular appointments. You also have 24/7 access to your care coordinator to help answer any questions you may have day or night.

Connected Care extends your care outside of the four walls of your physician's practice to ensure that you maintain the best possible health. Your Care Coordinator provides:

  • Personal attention from someone familiar with your health conditions

  • Assistance with scheduling appointments, lab tests or other tests

  • Explain how and when to take your medications

  • Coordinate any home health or medical equipment needs

  • Education on monitoring devices

  • Connects you with health education resources, services and programs

  • Identifies available community resources

  • Coordinates follow-up care after leaving the hospital

  • Provides holistic services that promote quality of life

  • Encourages you to reach your health goals

  • Provides access to other support services you need to stay well

FREQUENTLY ASKED QUESTIONS

  • Connected Care is a group of healthcare services that utilize a combination of healthcare professionals, personalized care plans, and technology to connect healthcare providers and patients, enabling remote monitoring, consultation, and treatment. The typical services include: Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Principal Care Management (PCM), Behavioral Health Integration (BHI), Remote Therapeutic Monitoring (RTM), Transitional Care Management (TCM), Chronic Pain Management (CPM), and others.

  • We will bill Medicare the allowable fee for connected care services once a month. Medicare will cover 80% of the charge and you will be billed for any applicable co-payment or deductible. In most cases If you have secondary or supplemental coverage the remaining 20% will be covered and you will have no out of pocket expense. Although you may or may not come into the office every month, your account will reflect this charge and you will be responsible for payment.

  • The main goal of the program is to empower patients by helping them manage their healthcare needs with the goal of improving their overall quality of life. Our dedicated nurses are available 24/7 to help you with your specific needs by assisting with the navigation of the ever-changing healthcare system.

  • We understand this is a common concern for our patients as they fear being taken advantage of and Medicare being overbilled. However, Medicare promotes this program due to the program's results of promoting better health and reducing overall health care costs by reducing emergency room and hospital visits.

  • Our nurses are available to discuss any issues or health concerns with any of your physicians to promote overall continuity of care. This reduces the burden for our patients as they no longer stress about informing all of their physicians of changes which in turn empowers them to focus solely on taking care of themselves.

  • No. Our patients still see their physician as scheduled or on an as needed basis. This program is an added service that allows the patients to better connect with their physician by keeping the physician updated on the patient's condition or needs month to month.

  • Just like with any other healthcare provider, the program and those providing services have a duty to protect the patients' data and ensure the security of data. The program complies with all privacy laws and regulations via the Health Insurance Portability and Accountability Act (HIPPA)
